All the qualifying calls must be placed into the Northern California Laser Center office (209) 549-2002 between December 1st and December 5th 2008.   Our office hours are 8 am - 5 pm. 

Website appointment requests are also accepted between Monday, November 24th, 2008 and Friday, December 5th, 2008.  Appointments must be confirmed by Wednesday, December 10th, 2008, 5 pm.

The complimentary consultation must be completed in the month of December 2008.

To qualify, all patients must be deemed as candidates by Amin Ashrafzadeh, MD (Dr. Ash).

A discount of $500 will be applied to the bilateral primary iLASIK procedures (both eyes).

The iLASIK procedures must be completed by January 16, 2009.

iLASIK procedure is the most advanced form of laser eye surgery using the IntraLase Blade-Free method along with CustomVue treatment using the Visx Star S4 IR.  This procedure is so advanced that it is the unique LASIK procedure allowed for TopGun pilots, Air Force pilots and the NASA Astronauts.
